What is Hallux Rigidus (Big Toe Joint Arthritis)

Hallux rigidus is a form of arthritis that affects the big toe joint. Over time, the joint becomes stiff, inflamed, and painful, especially when pushing off while walking. Patients often notice:

  • Difficulty bending the toe upward
  • Swelling or a bump on top of the joint
  • Pain with walking, running, or wearing certain shoes

Why Does It Happen?

This condition develops from wear and tear of the joint cartilage, but can also be related to:

As the cartilage wears down, the joint becomes stiff and may develop bone spurs, making motion even more limited and painful.

Joint Replacement (Arthroplasty) Surgery

Joint replacement of the big toe is an available option for certain patients, particularly those who want to maintain some motion in the joint. However, it is not our first-line surgical choice due to concerns about long-term durability, implant wear, loosening, and the potential need for revision surgery over time. In our experience, these factors can make outcomes less predictable compared to other procedures. That said, joint replacement can be appropriate in carefully selected patients, and we do offer this option when it aligns with a patient’s goals, anatomy, and activity level. Our priority is always to recommend the treatment that provides the most reliable and lasting result for each individual.
